Our Goal – To Improve the Health of the Patients and Communities We Serve

As part of our commitment to every patient, Broward Health provides health and wellness educational information – and prevention resources – to keep our patients healthy and well. For those who don’t have insurance or have minimal insurance coverage that doesn’t include some of the disease prevention screenings and tests, you can count on Broward Health to help. We offer free health screenings and community health fairs to learn more about nutrition and healthy habits. We’ll teach you how simple changes can make a big difference on your health and wellness.

Broward HealthPoint recognizes that to effectively care for its patients, its staff must go beyond the walls of Broward Health facilities and into the community. Many opportunities exist among community-based and faith-based organizations to introduce various healthcare initiatives. BHCHS Outreach Services staff plays an essential role in sharing that message.

Outreach Services’ primary objectives are to provide information about services available throughout Broward Health, educate the community on easy access to care and assist with referrals for services. BHCHS Outreach Services staff shares their knowledge and skills with the community to meet these objectives.

The Outreach Services Team coordinates free screenings in the community and promotes health and wellness educational programs. Many of these programs and services are offered at locations such as:

  • Community events
  • Health fairs
  • Homeowners associations
  • Apartment complexes
  • Community-based organizations
  • Childcare and senior centers
  • Faith-based organizations
  • Government organizations
  • Business associations
  • Schools/PTSAs

Our Key Focus Objectives:

  • Serve as a resource in the community
  • Implement a grassroots approach to marketing services close to home
  • Increase awareness of services available within Broward Health
  • Maintain dialogue with external agencies
  • Develop and maintain community partnerships

Nurse-Family Partnership

The Broward Health Nurse-Family Partnership maternal and early childhood health program introduces at-risk first-time parents to maternal and child health nurses who deliver the support first-time moms need to have a health pregnancy, become knowledgeable and responsible parents, and provide their babies with the best possible start in life. The relationship between mother and nurse provides the foundation for strong families, and lives are forever changed for the better.

Each new mom is assigned a registered nurse early in her pregnancy and receives ongoing nurse home visits during the term of the pregnancy. Nurse home visits continue with the young mother until the baby is two years old.

Broward Health's Nurse-Family Partnership is supported by the Florida Maternal, Infant and Early Childhood Home Visiting Initiative.

Healthy Families Program

Healthy Families Florida is a nationally accredited home visiting program for expectant parents and parents of newborns experiencing stressful life situations. The program improves childhood outcomes and increases family self-sufficiency by empowering parents through education and community support. The program promotes positive parent child relationships.

Broward Health's Healthy Families program is sponsored by the Ounce of Prevention Fund of Florida, the State of Florida Department of Children and Families, and Broward Health.

Broward Health provides Healthy Families screenings for infants born at Broward Health Medical Center and Broward Health Coral Springs to mothers from high risk zip codes. Through screening, Broward Health identifies families at risk and engages them with Healthy Families Broward to improve childhood outcomes.

Healthy Start

The Broward Health Healthy Start is a program that offers universal screening to all Florida pregnant women and their infants to ensure that early care is targeted to those families where there is the best opportunity of preventing or reducing of adverse health outcomes.

Broward Health's Healthy Start care coordination and educational program support families in reducing associated risk factors and toxic life stressors that place pregnant women and infants in jeopardy of having poor health outcomes. Services are provided based upon identified risk factors. The most frequently offered Healthy Start services are:

  • Breast feeding education and support
  • Care coordination/access to community services
  • Childbirth education
  • Interconceptional counseling and education
  • Parenting education and support
  • Tobacco education and cessation counseling

Any pregnant woman or child (birth to age three) is eligible for services. Services are provided at Broward Health Medical Center, Broward Health Coral Springs, Charles Drew Family Resource Center, Cora E. Braynon Family Health Center, Pompano Prenatal Care Center and Seagull School.

HIV/AIDS Services

Broward Health provides healthcare services for adult patients with HIV/AIDS at the Annie L. Weaver Health Center, Comprehensive Care Center and Specialty Care Center. Patient education and disease prevention are provided among clients and the community to promote reduction of high risk behaviors and compliance with treatment care plans. Referrals are available for specialty, dental care, mental health services and transportation.

Kinship Cares Initiative

A partnership between social service agencies, Legal Aid and Broward Health that employs nurses as healthcare navigators to assure that the healthcare needs of both children and their kinship caregivers are addressed. Partners who also assist kinship families include consumers, Broward County Health Department - KidCare, ChildNet, Children’s Services Council of Broward County, HANDY Inc., Kids in Distress, Legal Aid of Broward County, School Board of Broward County, The Pantry of Broward, Ounce of Prevention Fund of Florida, the United Way of Broward County, the Department of Children and Families, Friends of Children, Mental Health Association of Broward, and the Florida Kinship Center.

What is Kinship Cares?

The Kinship Cares Initiative (KCI) was created to improve the health – and healthcare – of kinship families. The program is operated by Broward Health, one of the nation’s largest healthcare systems.

Kinship Care is defined as any living arrangement that involves the care of a child(ren) by the following: an individual who is related to the child by blood/marriage, through the third degree, or through close family relationships that are acknowledged by the birth or adoptive parents.

Who is eligible?

Families that are living in kinship settings such as grandparents, an uncle or an aunt, a non-relative, or an adult sibling caring for children who are 0-18 years of age are eligible for enrollment in the program.

How will the program help?

The Kinship Cares Initiative (KCI) will promote and provide access to critical health services for children and their kinship caregivers. With the assistance of a medical case manager, the KCI will assist families with:

  • Establishing a medical home
  • Acquiring health insurance
  • Receiving wellness exams
  • Children’s immunizations

What will the Medical Case Manager do for the Kinship Family?

Provide a one-on-one personal health assessments for each family member

  • Help the family develop an individualized plan of care with health improvement goals
  • Teach the importance of taking medications
  • Provide nutritional education
  • Provide education to assist the family with understanding their medical needs
  • Assist with scheduling medical appointments
  • Empower the family to take control of their medical health through a primary care provider
  • Assist caregivers with understanding chronic healthcare conditions, such as hypertension and diabetes
  • Obtain medical attention for the children in their care
  • Provide assistance with navigating the healthcare system

What additional services does the Kinship Cares Initiative offer?

Families will be linked to social service agencies/programs to meet their non-medical needs, such as:

  • Case management
  • Educational support
  • Emergency financial assistance
  • Legal aid
  • Mental health linkage
  • Respite care
  • Other wrap around services
